1. Telephone Number

It is the first row in the MTNL surrender form. You have to fill in your MTNL telephone number that you want to close.

2. Name of the customer/company/organization (Surname first)

The MTNL broadband or telephone connection can be on your name or your business. When the MTNL connection is in your name, then you have to mention your name in the form. As per the form requirement, you have to write your last name first in the name row. You have to fill in the name of the person who took the MTNL connection.
